Forms ARE NOT on a first-come, first-serve; so please take your time, make sure all info is correct & fill out with as many details as possible.

TERMS & CONDITIONS

By submitting this form, you acknowledge and agree to the following:

  • Submission does not guarantee acceptance.

  • Books are open monthly. Please keep this in mind when submitting.

  • Ittybittyinkk will respond within 5-7 business days regarding the acceptance of your submission.

  • If your submission is accepted, you will receive a booking link.

  • A deposit is required at time of booking.

  • Due to high demand, the booking link will be active for 24 hours.

  • If your submission is not accepted, you will receive feedback on why.

  • New tattoo

  • Adding onto an existing tattoo that I have done

  • Those interested in both a new tattoo AND a touch-up on an existing tattoo that I have done.

    **Photo Requirement: Clients are required to submit a photo for an add-on or touch-up request.

ADDITIONAL INFORMATION BEFORE SUBMITTING…

There are Two Submission Forms to choose from. Please select the form based on your needs. The wrong form submitted may result in the denial of your submission.

TOUCH-UP FORM

  • Touch-ups on tattoos I have done

  • Requests are accepted for tattoos that are at least 4-6 weeks old but no older than 1 year.

    **Photo Requirement: Clients need to provide a photo of the tattoo they want to be touched up.
